Requirements

• Bachelor's degree in Social Work with 1+ years of community experience in health, medical, or homecare fields, or a Bachelor's degree in a related field with 2+ years of similar experience.
• Proficiency in Microsoft Office (Word, Excel) and adaptability to new software.
• Strong communication and interpersonal skills.
• Ability to handle sensitive information confidentially.
• Familiarity with care planning, community resources, and CAP/DA guidelines.

Responsibilities
