About The Position
The Case Manager is responsible for the care coordination of clients under the Assisted Living Waiver Program (ALWP) at Libertana. This role involves identifying, organizing, coordinating, and monitoring the needs of ALWP clients, ensuring their continued eligibility through regular follow-ups and compliance checks. The position requires excellent communication and organizational skills, as well as the ability to work independently in a hybrid/remote environment.
