About The Position

The Ambulatory Social Work Case Manager at the Lawrence Watkins Clinic is responsible for providing comprehensive care and services to patients and their families. This role involves collaboration with the healthcare team to ensure effective coordination of services, psychosocial assessments, and linkage to community resources. The case manager also provides emotional support and ensures that decision-makers are informed and involved in treatment planning, while adhering to state and federal regulations regarding documentation and reporting.

Requirements

  • Master's degree in Social Work from an accredited college or university.
  • Licensed Specialist Clinical Social Worker (LSCSW) or Licensed Masters Social Worker (LMSW) in the State of Kansas.
  • Basic computer skills.

Responsibilities

  • Accept responsibility and accountability for optimal outcomes within their scope of practice.

  • Assume responsibility for risk and safety issues associated with the position.

  • Acquire and maintain knowledge and competence related to the position's expectations.
  • Complete psychosocial assessments of patient/family situations, including social, psychological, emotional, and financial factors.
  • Evaluate psychosocial and medical/nursing information to determine appropriate social service action/plan of care.
  • Participate in interdisciplinary team meetings and family conferences to assess needs.
  • Advocate for patients and caregivers to access services and protect their rights.
  • Provide supportive counseling to assist patients/families in adjusting to disability and illness.
  • Document appropriate information in the patient's medical record to ensure communication of psychosocial needs.
  • Work in partnership with physicians and clinic nurses to ensure timely linkage to services.
  • Participate in professional development activities and assist in training new team members.
