About The Position
The position involves intervening with patients and families to address psychosocial needs and provide assistance and support in coping with chronic, acute, and terminal illnesses. Services offered may include consultation, discharge planning, education, counseling, advocacy, and information referrals to meet the emotional, social, and financial aspects of care. The role also includes providing crisis intervention to patients and families with psychosocial needs and coordinating the development of a discharge plan of care. The professional will provide patient and family services through case finding referrals from physicians and healthcare team members, acting as an expert and liaison between patients and community resources. Additionally, the position requires identifying patient and family psychosocial and environmental needs related to admission, diagnosis, treatment, and discharge, and working collaboratively with other members of the interdisciplinary team to develop and implement a comprehensive discharge plan. The role may involve making appropriate referrals to community services, providing inpatient emotional support and counseling, crisis intervention, and bereavement services. The professional will also assess and intervene in situations involving child or elder abuse/neglect and domestic violence, providing care and services to all patient groups, including neonatal, infant, pediatric, adolescent, and adult populations.
