The position involves intervening with patients and families to address psychosocial needs, providing assistance and support in coping with chronic, acute, and terminal illnesses. Services offered may include consultation, discharge planning, education, counseling, advocacy, and information referrals to meet the emotional, social, and financial aspects of care. The role also includes providing crisis intervention to patients and families with psychosocial needs and coordinating the development of a discharge plan of care. Patient and family services are provided through case finding referrals from physicians and healthcare team members. The individual will serve as an expert and act as a liaison between patients and community resources, identifying psycho-social and environmental needs related to admission, diagnosis, treatment, and discharge. Collaboration with other members of the interdisciplinary team is essential in developing and implementing a comprehensive discharge plan. The position may also involve providing inpatient emotional support, counseling, crisis intervention, and bereavement services. Additionally, the role includes assessing and intervening in situations involving child or elder abuse/neglect and domestic violence, providing care and services to all patient groups including neonatal, infant, pediatric, adolescent, and adults.
