The Social Work Assistant at MidHudson Regional Hospital plays a crucial role in providing case management and discharge planning services within the Case Management department. This position is integral to the multidisciplinary treatment team, actively participating in daily meetings to discuss patient care and treatment plans. The Social Work Assistant collaborates with various stakeholders, including treatment teams, patients, families, and community agencies, to develop and support individualized discharge plans that cater to the unique needs of each patient. In this role, the Social Work Assistant is responsible for obtaining necessary consent forms from patients to authorize the disclosure of protected health information (PHI) to relevant parties, including family members, community case managers, and healthcare providers. The assistant also supports both the inpatient and Emergency Department by making outreach calls to families and community-based organizations to ensure a smooth discharge process. This includes sending referral packets and scheduling follow-up appointments for aftercare, as well as maintaining accurate documentation of all discharge planning activities. The Social Work Assistant will complete Single-Point of Access (SPOA) applications, follow up on their status, and assist in the completion of applications for state hospitals, Assertive Community Treatment (ACT) Teams, and Assisted Outpatient Treatment (AOT). Proficiency in computer applications is essential, as the assistant will utilize spreadsheets, word processing, and email to perform work assignments efficiently, typing at a minimum speed of 50 words per minute. Additionally, the Social Work Assistant will participate in various departmental projects, including the Delivery System Reform Incentive Payment Program (DSRIP), and contribute to staff meetings, case conferences, and training activities. Establishing and maintaining effective interpersonal relationships with patients, caregivers, and treatment team members is vital for success in this role. The position may also involve other duties as assigned, ensuring a comprehensive approach to patient care and support.
