About The Position

The Social Work Assistant 2 position at the Office for People With Developmental Disabilities involves providing a wide variety of social services and support for assigned individuals. The role includes independently conducting written and verbal assessments, monitoring individual programs, responding to emergencies, and participating in treatment team meetings. Responsibilities also include completing necessary paperwork for admissions and discharges, providing individual counseling, advocating for individuals' needs, coordinating family visits, and ensuring proper documentation of services delivered. Requirements

  • A bachelor's degree in a human services field and two years of social work experience.
  • One year of service as a Social Work Assistant 1 for promotion.
  • A master's degree in a human services field can substitute for one year of specialized experience.

Responsibilities

  • Performs a wide variety of social services and support for assigned individuals.
  • Independently provides written, verbal assessments, and service programs.
  • Monitors individual's programs to ensure continuity, consistency and progress.
  • Responds to emergency situations as needed.
  • Attends and participates in treatment team meetings.
  • Completes admission and discharges of individuals including required paperwork.
  • Completes and submits 30-day, quarterly and annual assessments.
  • Provides individual counseling sessions as needed.
  • Ensures and provides documentation of services delivered.
  • Advocates for the needs of the individuals.
  • Coordinates and arranges family visits.
  • May accompany individuals on family visits.
  • Obtains, monitors, and follows up on consents for RRC and ICC.
  • Functions as a liaison between individual's families and the team.
  • Obtains informed consent from individuals/families/legal guardians.
  • May teach classes.
  • Completes and uploads MMAs and PEPs.
  • Compiles LRAC/SFAC packets and community referral packets.
  • Assists with self-advocacy group.
  • Other duties as assigned.
