About The Position

The Social Work Assistant 2 (NY HELPS) position is a full-time, permanent role within the Office for People With Developmental Disabilities. The incumbent will serve as a housing liaison on the regional capacity management team, managing a caseload of individuals seeking placement in certified residential opportunities. This role involves coordinating with various stakeholders to ensure individuals receive appropriate placements and services based on their needs.

Requirements

  • Bachelor's degree in a human service field and 2 years of social work experience, or a Master's degree in social work which can substitute for 1 year of experience.
  • Candidates within State Service must be reachable on the current eligible list for competitive class appointment.
  • Candidates must have one year of permanent competitive or 55-b/55-c service in a title eligible for transfer under Section 70.1 of the Civil Service Law.

Responsibilities

  • Manage a caseload of individuals seeking placement in certified residential opportunities.
  • Review individual's referral clinical information to understand their needs and follow up for updated information as necessary.
  • Monitor residential vacancies and make referrals to appropriate residential programs.
  • Interface with care managers, hospitals, and nursing homes regarding referrals for placement.
  • Track referrals and follow up with agencies using the SharePoint system to ensure timely screenings and outcomes reporting.
  • Run Access to Residential Options Committee (AROC) meetings with providers.
  • Coordinate with Care Managers and CSIDD to discuss individual needs during the placement process.
  • Review DDP2 scores and advise potential providers on acuity levels, re-administering instruments as necessary.
  • Enter information into the Residential Opportunities database and SharePoint systems.
  • Assist in finding placements for individuals aging out of residential schools identified as emergency needs.
