About The Position

The Social Worker position at Parkside Health Care Center involves organizing, developing, and coordinating various aspects of resident care. The role requires conducting initial and ongoing assessments to meet the needs of residents and acting as a liaison between residents, their families, and staff to address any concerns that may arise.

Requirements

• At least one year of experience in Social Service work in a long-term care setting preferred but not necessary.
• Must be willing to work flexible hours.
• Must have excellent communication skills, both written and verbal.
• LSW or a degree in social work required; a degree in a similar field with experience in long-term care will be considered.

Responsibilities

• Organizing, developing, and coordinating aspects of resident care.
• Conducting initial and ongoing assessments required to meet resident needs.
• Acting as a liaison between the resident, family, and staff regarding concerns.
