About The Position

The Acute Care Clinical Social worker provides social work services to patients and their families maximizing utilization of personal, family, community and health care resources to formulate an appropriate safe discharge plan in the acute care setting. Works with case management to have an understanding of the patients utilization plan. Evaluates or assists the attending physician/interdisciplinary team in evaluating the psychosocial needs of the patient. Works collaboratively with case manager and the interdisciplinary team in developing and implementing a comprehensive integrated discharge plan in the acute care setting. Completes initial and ongoing assessment of the patient and families response. Assesses the capacity of the home caregiver to cope with the patient's needs in the development of the plan of care. Completes appropriate psychosocial assessment and referrals. Identifies patients requiring crisis intervention and acts as soon as possible to resolve the issue(s) and prevent barriers to patient flow. Conducts a comprehensive patient/family assessment. Assesses the patients third party payer coverage to facilitate development of a post-acute discharge plan. Clearly documents in the medical record the discharge plan and updates on an ongoing basis. Provides ongoing education to patients and families as it relates to care planning and preparation for discharge. Maintains systems to accurately track and trend variances in care and service. Role perceived as leader in support of psychosocial in treatment and outcome. Serves as role model, sharing knowledge base and practice standards by participation in a formal social work internship program as a preceptor as requested. Follows advanced learning programs guidelines and gives feedback into the development of the social worker intern. Serves as departmental preceptor for new social work staff promoting quality care monitoring and evaluating performance for areas of improvement.
