Requirements

  • Bachelor’s degree in a healthcare-related field preferred
  • Minimum of 2 years’ experience in social work, case management, or a related field
  • Experience with discharge planning or care transitions in post-acute or acute care settings preferred
  • Strong communication, organization, and documentation skills
  • Ability to remain calm, flexible, and compassionate in emotionally charged situations
  • Comfort using technology in a fast-paced healthcare environment
  • Ability to lift up to 35 pounds
  • Ability to sit, stand, bend, and move throughout the day
  • Must be able to read, write, and speak English
  • Possible exposure to communicable diseases and bodily fluids

Nice To Haves

  • Bachelor’s degree in a healthcare-related field preferred
  • Experience with discharge planning or care transitions in post-acute or acute care settings preferred

Responsibilities

  • Meeting with patients and families at admission to complete paperwork and identify psychosocial needs
  • Coordinating care transitions from admission through discharge in collaboration with the interdisciplinary team
  • Completing referrals, assessments, care plans, progress notes, and required discharge documentation
  • Communicating care recommendations and transition plans clearly with patients and families
  • Providing counseling, education, and advocacy to protect patient rights and support decision-making
  • Assisting with insurance-related matters including Medicare/Medicaid notifications, denials, and copays
  • Responding to patient and family concerns promptly and professionally
  • Educating care teams on identifying social service needs and available resources
  • Participating in committees and meetings as assigned
