Requirements

• Required Bachelors in Social Work, Sociology, or Related Field
• Required 6 months Internship or 6 months Social Work in an acute care setting

Nice To Haves

• Preferred ACM (Case Management)
• Preferred CCM (Case Manager)
• Preferred Masters Degree in Social Work, Sociology or Related Field (If MSW level, will be hired as Social Services Specialist II)

Responsibilities

• Being a member of a collaborative practice model that includes patients, nurses, hospital care coordinators, physicians, and other practitioners, caregivers, and the community
• Provides coordination, referral and interdisciplinary discharge planning
• Knowledgeable about treatment, social and economic implications for individuals and families as it pertains to their age, stage of development and discharge needs
• Possesses a clear understanding of discharge and transition planning and linkage with community resources
• Demonstrates skills in planning, organizing, and managing multiple functions
