Requirements

• Bachelor's or Master's in Social Work or related field (required)
• 3+ years' experience in long-term care, skilled nursing, or healthcare (required)
• 1 year of supervisory experience preferred
• Licensed Clinical Social Worker (LCSW) or Licensed Master Social Worker (LMSW) preferred
• Strong leadership, communication, and organizational skills

Responsibilities

• Lead and oversee all functions of the Social Services Department in our Health Care Center.
• Supervise and mentor a compassionate team, ensuring top-quality care and compliance with state and federal regulations.
• Provide direct social work services to residents, guiding care plans and psychosocial support.
• Coordinate discharge planning and build partnerships with community agencies.
• Advocate for resident rights and emotional well-being.
• Conduct audits, develop programs, and drive quality improvement initiatives.
• Collaborate closely with nursing, therapy, chaplaincy, and other departments to deliver holistic care.

Benefits

• Starting Salary: $65,788-$72,366.80/year (DOE)
• Schedule: Monday-Friday 8:00a-4:30pm, every other weekend on-call rotation, holidays, evenings and weekends as needed
