The part-time social worker will support the Social Services and MDS Departments ensuring accurate and timely assessments of residents in a long-term care facility, which helps determine care plans and reimbursement levels. This role requires strong clinical skills, attention to detail, helping residents and their families address psychosocial, emotional, and practical concerns, as well as assisting with discharge planning, community resource referrals, care coordination and the ability to collaborate effectively with the interdisciplinary care team to provide the highest quality of care for residents. The Social Services / MDS Assistant works closely with the Social Services Director, MDS Director, and Director of Nursing as well as other healthcare professionals, and residents to ensure that residents' overall well-being and quality of life are maintained.
