About The Position

Social Services In-Training JOIN AN OUTSTANDING TEAM! Marquis Companies is looking for passionate individuals who are dedicated to improving the lives of seniors. As a participant in our Social Services In-Training Program, you will receive paid, hands-on mentorship and structured training to prepare you for a long-term career in social services within our Skilled Nursing Facilities (SNFs). In this role, you’ll help address the complex psychosocial needs of our residents and assist short-term patients with comprehensive discharge planning. You’ll also help ensure safe transitions back into the community by connecting individuals and families with vital resources and services. What Does our Training Program Look Like? Takes place at one or more of our Post-Acute Rehab facilities in the Portland Metro Area Includes training on a variety of shifts to expose you to different aspects of social services operations Program length varies depending on individual learning progress, typically 3-6 months Upon completion, you will be eligible to apply for open Social Services roles within Marquis Companies For more than 30 years, Marquis Companies and Consonus Healthcare have been serving seniors and welcoming staff of all backgrounds, skills, and perspectives. Requirements

  • Associate or bachelor’s degree in one or more of the following areas is required
  • Behavioral Sciences (Human Development, Psychology, Sociology, Counseling), Healthcare Administration and/or Health Sciences
  • Passion for working with seniors and supporting their care journeys

Responsibilities

  • Learn how to manage complex psychosocial needs and discharge planning
  • Collaborate with inter-disciplinary care teams and external providers
  • Complete resident assessments (MDS), person-centered care plans, and insurance coordination
  • Gain experience with our Electronic Medical Records (EMR) system
