About The Position

The Social Services Director is responsible for providing medically related social services to residents, ensuring their physical, mental, and psychosocial well-being. This role involves establishing care plans, assisting residents with health care decisions, and coordinating with community resources to meet residents' needs. The position requires strong communication skills and the ability to handle emotional support for residents and their families.

Requirements

• Bachelor's Degree in Social Work or Human Services.
• 2 years of supervised social work experience in a health care setting working directly with individuals.
• Knowledge and experience with PCC preferred.

• MSW preferred.

Responsibilities

• Establish course of action by exploring options and setting goals with residents and their families.
• Assist residents in achieving the highest practicable level of self-care, independence, and well-being.
• Provide medically related social services to maintain residents' physical, mental, and psychosocial well-being.
• Evaluate social and family information to assess social services needs and develop care plans.
• Provide emotional support and address emotional problems for residents and families.
• Assist residents with health care decisions and personal shopping.
• Track and inventory patient belongings, coordinating responses to reports of missing items.
• Assist in obtaining resources from community and social services agencies.
• Secure appropriate prosthetics and assistive devices for residents.
• Develop and maintain relationships with other departments and community agencies.
• Document resident social service status and assist in making outpatient appointments.
• Assist in discharge planning with appropriate agencies and individuals.
• Coordinate with the interdisciplinary team and provide in-service training to staff.
• Counsel residents and family members.
