You’ll advocate for residents, guide families, coordinate admissions and discharges, and ensure compliance with state and federal regulations.

Requirements

  • Master’s Degree (strongly- preferred) or Bachelor’s degree in Social Work or equivalent experience
  • Knowledge of admissions, discharge planning, and regulatory requirements
  • Strong communication, organizational, and interpersonal skills

Nice To Haves

  • Prior experience in short-term or long-term care strongly preferred

Responsibilities

  • Provide ongoing resident and family counseling, including crisis intervention and adjustment support.
  • Complete psychosocial assessments, quarterly reviews and MDS as required.
  • Coordinate discharge planning, follow-ups, and post-discharge calls within 48 hours
  • Assist residents and families with placement options and transitions
  • Participate in care plan conferences, resident council meetings, and interdisciplinary team meetings
  • Oversee admissions support, facility tours, and marketing/networking efforts
  • Manage PASRR, MA-51s, 408s, MDS sections, and required documentation
  • Address resident and family concerns promptly and professionally
  • Lead and oversee social services operations
  • Maintain department policies, procedures, and compliance standards
  • Directly supervises assigned personnel for dept.
