You’ll advocate for residents, guide families, coordinate admissions and discharges, and ensure compliance with state and federal regulations.

Requirements

Master’s Degree (strongly- preferred) or Bachelor’s degree in Social Work or equivalent experience
Knowledge of admissions, discharge planning, and regulatory requirements
Strong communication, organizational, and interpersonal skills

Nice To Haves

Prior experience in short-term or long-term care strongly preferred

Responsibilities

Provide ongoing resident and family counseling, including crisis intervention and adjustment support.
Complete psychosocial assessments, quarterly reviews and MDS as required.
Coordinate discharge planning, follow-ups, and post-discharge calls within 48 hours
Assist residents and families with placement options and transitions
Participate in care plan conferences, resident council meetings, and interdisciplinary team meetings
Oversee admissions support, facility tours, and marketing/networking efforts
Manage PASRR, MA-51s, 408s, MDS sections, and required documentation
Address resident and family concerns promptly and professionally
Lead and oversee social services operations
Maintain department policies, procedures, and compliance standards
Directly supervises assigned personnel for dept.
