About The Position

SOCIAL SERVICES DIRECTOR, Generations Applewood, Matteson, IL Generations at Applewood, in Matteson, IL, has an Opening for a Social Services Director. Requirements

  • BA or Master’s Degree in Social Work (MSW) - or - BA in Psychology or Sociology
  • A member of the National Association of Social Workers (NASW) and Academy of Certified Social Workers (ACSW) desirable
  • Minimum two years' supervisory experience
  • A minimum of three (3) years of experience in field of social services in a hospital, nursing or long-term care facility
  • Demonstrated knowledge of the social dynamic of the geriatric population
  • Knowledge of community resources
  • Initiative to seek new methods and services for residents
  • Strong leadership and relationship building skills
  • High level of organizational professionalism
  • Working knowledge of government regulations as relevant to social services

  • A member of the National Association of Social Workers (NASW) and Academy of Certified Social Workers (ACSW) desirable

Responsibilities

  • Assist with planning, developing and directing our facility’s social service programs in accordance with current federal, state and local standards, guidelines and regulations
  • Ensure that medical, emotional, and social needs of each resident is met/maintained
  • Greet and orient new residents
  • Provide information regarding community resources to residents' families
  • Direct hospice and palliative care
  • Coordinate discharge planning
  • Facilitate regular care plan meetings with residents and their families
  • Complete the MDS (Minimum Data Set) within the Assessment Reference date
  • Address residents' grievances and assist nursing or other pertinent departments in resolving issues
  • Attend/facilitate intervention, rehabilitation, marketing, performance improvement and Medicare meetings as appropriate
