Requirements

• Bachelor’s Degree in Social Work or related Human Services field
• At least one (1) year of social services experience in a healthcare setting
• Strong communication and interpersonal skills
• Ability to lead, organize, and prioritize in a fast-paced environment
• Knowledge of regulatory requirements and resident rights
• Compassion, empathy, and a commitment to quality resident care

Responsibilities

• Lead and oversee the Social Services Department to support residents’ emotional and psychosocial well-being
• Develop and implement individualized care and discharge plans
• Partner with residents, families, and the care team to ensure needs are met
• Provide guidance, support, and crisis intervention to residents and families
• Coordinate referrals to community resources and services
• Ensure compliance with all federal, state, and facility regulations
• Maintain accurate documentation, care plans, and social histories
• Supervise and support Social Services staff
• Participate in interdisciplinary team meetings and quality assurance initiative
