About The Position

The Social Services Director plays a vital role in supporting residents and families throughout their journey in our community. This position provides compassionate social services and admissions assistance, helping ensure each resident's emotional, social, and psychosocial needs are met with dignity, respect, and individualized care. Serving as an advocate, counselor, and resource, the Social Services Director helps create a welcoming, supportive environment where residents feel heard, valued, and at home. This position is more than coordination and documentation - it's about listening, advocating, and helping residents feel safe, supported, and respected every day. You'll be part of a compassionate team dedicated to creating meaningful connections and improving quality of life for every resident.

Requirements

  • Bachelor's or Master's in Social Work or related field
  • Experience in social services, behavioral health, or long-term care
  • Strong communication, leadership, and relationship-building skills

Responsibilities

  • Guide residents and families through the admissions process, providing education, reassurance, and emotional support
  • Meet with new residents to assist with adjustment, address concerns, and provide counseling as needed
  • Educate residents and families on resident rights, responsibilities, and available services
  • Build trusting, meaningful relationships with residents and families, serving as a consistent point of support
  • Complete social histories, psychosocial assessments, and required documentation, including the MDS social services section
  • Participate in interdisciplinary care planning to ensure resident goals, preferences, and needs are reflected in care plans
  • Monitor resident well-being and advocate for care plan updates as needs change
  • Support residents during room changes, care transitions, and life adjustments with compassion and follow-up
  • Identify changes in resident behavior or emotional status and communicate concerns promptly to the care team
  • Serve as discharge coordinator, developing safe, realistic discharge plans in partnership with residents, families, and community agencies
  • Connect residents and families with community resources and services that support continued well-being
  • Establish and maintain positive relationships with community agencies and referral partners
  • Represent the facility with warmth and professionalism to community groups and referral sources
  • Advocate for resident rights and investigate concerns or allegations, reporting findings appropriately
  • Coordinate legal status changes such as guardianships and protective placements
  • Ensure services are delivered in compliance with applicable regulations and standards
