The role involves planning, organizing, developing, and directing social service programs to ensure that the medical-related emotional and social needs of residents are met. You will assess and identify these needs, develop and implement social care plans, and consult with healthcare team members regarding psychosocial factors related to residents' illnesses. Additionally, you will provide discharge planning and assist residents with financial and legal matters. This position also includes direct supervision of staff and ensuring compliance with federal and state regulations.

  • Plans, organizes, develops, and directs social service programs.
  • Assesses and identifies medical-related social and emotional needs of residents.
  • Develops and implements social care plans with involvement from residents, families, and healthcare staff.
  • Consults with physicians and healthcare team members regarding psychosocial factors.
  • Provides discharge planning and referrals to community and family social services.
  • Assists residents with financial and legal matters.
  • Supervises staff, including interviewing, hiring, and training employees.
  • Monitors supply and equipment needs and budget.
  • Implements policies and procedures to ensure compliance with regulations.
  • Develops and maintains community liaison activities.
  • Bachelor's degree (B.A. or B.S.) in Social Work from a four-year college or university.
  • Minimum of two years related supervisory experience and/or training.
  • ACSW registration.
