You will assess and identify these needs, develop and implement social care plans, and consult with healthcare team members regarding psychosocial factors related to residents' illnesses. Additionally, you will provide discharge planning and assist residents with financial and legal matters. This position also includes direct supervision of staff and ensuring compliance with federal and state regulations.
