About The Position

As the Social Services Designee at Laurel Health Care Company, you will provide medically-related social services that help guests to attain and/or maintain their physical, mental and psychosocial well-being. The role is dedicated to improving the lives of seniors and involves working closely with the facility's Social Service Department to streamline the discharge planning process.

Requirements

• High school diploma/GED.

Responsibilities

• Assist and coordinate with the facility's Social Service Department for discharge planning.
• Meet the resident/guest on or shortly after admission and follow them until discharge.
• Attend the 72 hour care plan meeting.
• Order equipment for the community setting.
• Arrange home health nursing for after discharge.
• Coordinate medications for community use.
• Schedule IV home services if necessary.
• Communicate with external case managers as needed.
• Coordinate the entire discharge with IDT involvement.
• Attend care conferences for sub-acute residents/guests.
• Develop an individualized discharge care plan in collaboration with the facility, family, and practitioner.
