About the position
As the Social Services Designee at Laurel Health Care Company, you will provide medically-related social services that help guests to attain and/or maintain their physical, mental and psychosocial well-being. The role is dedicated to improving the lives of seniors and involves working closely with the facility's Social Service Department to streamline the discharge planning process.
Responsibilities
- Assist and coordinate with the facility's Social Service Department for discharge planning.
- Meet the resident/guest on or shortly after admission and follow them until discharge.
- Attend the 72 hour care plan meeting.
- Order equipment for the community setting.
- Arrange home health nursing for after discharge.
- Coordinate medications for community use.
- Schedule IV home services if necessary.
- Communicate with external case managers as needed.
- Coordinate the entire discharge with IDT involvement.
- Attend care conferences for sub-acute residents/guests.
- Develop an individualized discharge care plan in collaboration with the facility, family, and practitioner.
Requirements
- High school diploma/GED.
