Requirements
- Possess strong computer skills and social skills
- Possess strong time management and organizational skills
- Desire for a short-term temporary role
Responsibilities
- Pre-admission screenings, assessments, discharge planning
- Schedule and mediate multidisciplinary care plan meetings
- Collaborate with the team to meet the individual psychosocial needs of our residents
