Your day to day as a Social Services Designee As a Social Services Designee at Crandall Medical Center, you will act as a residents' advocate, invite family members to care plan meetings, participate in nursing care conferences, coordinate discharge plans for residents, conduct and participate in quality assurance studies, p erform duties for resident shopping program and much more. Engaging with residents and their families will be a key aspect of your role, fostering a supportive and empathetic atmosphere throughout the facility. Requirements

  • Associates degree in Social work or equivalent field
  • Social Services Certificate preferred
  • Two years of experience working in an office
  • Ability to pass a Physical
