About the position
Citizens Memorial Healthcare Foundation (CMH) is recruiting for a Social Worker/Social Service Designee to join the dynamic team of providers already in place providing exceptional patient care at the Lake Stockton Healthcare Facility. The skilled nursing facility is a 90-bed facility located in Stockton, MO that includes two memory care units for individuals with Alzheimer's disease and dementia. A typical day will include: Provides Social Services to residents; You will stay with residents throughout their stay at the facility and will be responsible for assisting with the completion of the admission process and working with the interdisciplinary team on discharge planning. Actively and consistently contributes to facility operations communications; behaves in a manner consistent with the mission and objective of CMH and performs other duties as requested.

Requirements
- Bachelor's degree in Social Work or related field preferred
- MHCA Designee Training required
