As a Social Services Coordinator , you will play a vital role in supporting the emotional, social, and discharge planning needs of our residents within a skilled nursing setting. You’ll collaborate closely with interdisciplinary teams, residents, and families to ensure individualized care plans are followed, resources are coordinated, and each resident experiences a compassionate, respectful, and supportive environment.
