About The Position
The position is responsible for maintaining up-to-date evaluation documentation on each Resident’s activities at the facility, ensuring compliance with Federal, State, and Local regulations. The role involves attending scheduled care conferences to identify and interpret the psychosocial needs of Residents for inclusion in their care plans, addressing various issues that reinforce individuality and identity. The position also includes assisting with coordination of intake, admission, and Resident transfers within the facility, as well as inspecting Resident belongings to ensure proper labeling and inventory. Additionally, the role requires attending interdisciplinary team meetings, assessing Residents for psychosocial groups, and supervising the implementation and documentation of these assessments. The individual will mediate issues among Residents, families, and staff, ensure completion of DPOA or guardianship paperwork, and coordinate services with psychiatric providers and OBRA. The position also involves providing linkage with community resources, developing comprehensive discharge plans, and offering informal counseling to assist Residents in socialization and participation in activities. The individual will maintain a filing system for the Social Services Department, operate within budgetary guidelines, report hazardous conditions, and ensure infection control practices are followed. The role emphasizes maintaining the comfort, privacy, and dignity of Residents while promoting a caring environment and effective communication with all stakeholders.
