About The Position
The Social Services Coordinator is responsible for ensuring that residents' psychosocial and concrete needs are identified and met in accordance with federal, state, and company requirements. This role involves orienting residents to the center, planning for discharges, and maintaining comprehensive records of care and referrals. The coordinator also plays a key role in communicating with residents, families, and staff to ensure a high standard of care and service delivery.
