About The Position

The Social Services Coordinator in Long Term Care (LTC) serves as a member of the interdisciplinary team, providing assistance with activities, social, emotional, and economic concerns of the resident and family. This role enables residents to achieve or maintain an optimal level of functioning. The coordinator utilizes appropriate age-related resident care protocols relating to the physical and psychological needs of adult and geriatric patients/residents. They work closely with the interdisciplinary team to achieve sound and timely outcomes, may formulate care plans, facilitate care conferences, investigate grievances, and complete assessments as assigned. The position requires a working knowledge of teamwork principles and the impact of illness on residents and families, along with clinical experience in psychosocial and economic management of common problems associated with illness.

Requirements

  • Minimum high school graduate or equivalent required.
  • Prior experience in long term care, social services, or human services preferred.
  • Certified Nursing Assistant (CNA) certification required within six months of hire based on location.

Responsibilities

  • Provide assistance with activities, social, emotional, and economic concerns of residents and families.
  • Utilize age-related resident care protocols for adult and geriatric patients/residents.
  • Work closely with the interdisciplinary team to achieve timely outcomes.

  • Align resources to support resident care.

  • Exhibit proficiency in verbal and written communication.
  • Establish rapport with residents, families, healthcare team, and community.
  • Demonstrate critical thinking skills for independent judgment and active participation with the healthcare team.
