About The Position
The Social Services Coordinator evaluates the social needs of each resident admitted and formulates a written plan of care; reviews and revises plan as necessary, but no less often than quarterly. Refers residents and families to appropriate community resources as necessary. Carries out assigned duties and responsibilities in accordance with current existing federal and state regulations and established organization policies and procedures for all Health Center residents and families.
