About The Position

Esperanza Health Center is seeking a bilingual Social Service Consultant (SSC) to join our interdisciplinary care team. Social Service Consultants work directly with patients—both in person and by phone—to assess social needs, connect patients to critical community resources, and reduce barriers to care. This position is grant-funded for an anticipated three-year period, pending final grant approval with continued employment contingent upon ongoing funding availability and organizational needs. SSCs collaborate closely with medical providers, behavioral health clinicians, and community health staff to deliver comprehensive, patient-centered care addressing social determinants of health in a culturally responsive and holistic manner. Hourly Rate: For candidates with their bachelor's and higher: 19.50-22.50/hour For candidates with their HS Diploma: 17.00-20.00/hour

Requirements

  • Bachelor’s degree preferred
  • Bilingual in English and Spanish required
  • Ability to work effectively in a fast-paced, team-based environment
  • Strong communication, organization, and interpersonal skills
  • Passion for innovative approaches to delivering high-quality social services
  • Commitment to working in a Christian healthcare ministry grounded in compassion, dignity, and service

Responsibilities

  • Meet with patients in person or by phone to assess social determinants of health and provide culturally appropriate resources
  • Maintain accurate documentation in the Electronic Medical Record (EMR)
  • Provide limited case management services for an assigned panel of patients living with HIV
  • Provide crisis intervention and referrals for patients experiencing domestic violence or child abuse
  • Conduct HIV counseling and testing in accordance with established guidelines
  • Assist uninsured patients with enrollment in health insurance programs and follow up to address barriers
  • Support patients with insurance renewal processes as needed
  • Facilitate referrals to Early Intervention and Autism Evaluation services
  • Work with children and their families to support linkage to needed resources and assist with navigating the educational system, welfare system, and other external agencies impacting health and well-being
