About The Position

The Social Services Assistant plays a crucial role in supporting the Social Service Director/Consultant in the planning, development, and implementation of social service programs within the facility. This position focuses on enhancing the well-being of residents by addressing their social and emotional needs, coordinating with various departments, and ensuring compliance with policies and procedures.

Requirements

  • Experience in social services or a related field.
  • Knowledge of Medicare/Medicaid and financial assistance programs.

Responsibilities

  • Assist the Social Service Director/Consultant in planning and implementing social service programs.
  • Meet with administration, medical and nursing staff to plan social services as directed.
  • Review and recommend changes to department policies and procedures annually.
  • Develop and implement policies for identifying residents' social and emotional needs.
  • Participate in community planning related to facility interests and resident needs.
  • Assist residents in achieving self-care, independence, and well-being.
  • Provide medically related social services to maintain residents' physical, mental, and psychosocial well-being.
  • Complete necessary administrative forms and reports for the Director.
  • Provide information to residents and families about Medicare/Medicaid and financial assistance programs.
  • Consult with staff and community agencies to address residents' needs through social service programs.
  • Record and maintain Social Service progress notes regarding treatment plans and institutional adjustments.
  • Coordinate social service activities with other departments as necessary.
  • Assist residents and families with emotional problems related to illness and institutional care.
  • Evaluate social and family information to determine social treatment plans.
  • Meet regularly with department personnel to identify and improve service areas.
  • Obtain resources from community agencies to meet residents' needs.
  • Assist in scheduling outpatient appointments and on-site ancillary services.
  • Participate in discharge planning with appropriate agencies and referrals.
  • Develop and maintain rapport with other departments and community agencies.
