We are seeking a compassionate and organized Social Services Assistant to join our team and make a positive impact. The Social Services Assistant supports the social and emotional well-being of our residents, helps with admissions, discharge planning, and ongoing social work services. This role involves building trusting relationships with residents and their families, ensuring they receive the necessary resources and support.

Requirements

• Bachelor’s degree in Social Work, Psychology, Sociology, or a related Human Services field.
• Previous experience in a healthcare, senior living, or social services setting preferred.
• Strong interpersonal and communication skills.
• Ability to work collaboratively with residents, families, and interdisciplinary teams.

Responsibilities

• Welcome new residents and assist with their adjustment.
• Develop individualized care plans addressing psychosocial needs.
• Conduct comprehensive social histories and assessments.
• Participate in interdisciplinary care planning and resident conferences.
