About The Position

Visalia Post Acute is seeking a compassionate and detail-oriented Social Services Assistant (SSA) to join our team. This role will support our Social Services Director/Consultant in ensuring our residents receive the highest level of social, emotional, and psychosocial care.

Requirements

  • Prior experience working in a Skilled Nursing Facility (SNF).
  • High School Diploma or equivalent (Associate/Bachelor’s Degree preferred).
  • Strong communication, organizational, and problem-solving skills.
  • Ability to read, interpret, and apply policies and procedures.
  • Compassionate and professional demeanor with residents, families, and staff.
  • Physical ability to occasionally lift/move up to 25 lbs and assist during emergencies.

Nice To Haves

  • Associate/Bachelor’s Degree in a related field.

Responsibilities

  • Assist the Social Service Director/Consultant in planning, developing, organizing, and evaluating social service programs.
  • Meet with administration, medical/nursing staff, and other departments in planning and coordinating social services.
  • Assist with policy and procedure development, review, and annual updates.
  • Help identify and address residents’ medically related social and emotional needs.
  • Participate in community planning and build relationships with outside health, welfare, and social agencies.
  • Provide support to residents in achieving independence, self-care, and overall well-being.
  • Offer guidance to residents and families regarding Medicare/Medicaid and other financial assistance programs.
  • Document and maintain accurate Social Service progress notes and reports.
  • Provide staff consultations, assist with discharge planning, and coordinate on-site ancillary services (optometry, podiatry, dentistry, psychiatry, etc.).
  • Support residents and families with emotional challenges such as illness, adjustment to care, and end-of-life needs.
  • Assist in obtaining resources from community agencies and making outpatient appointments.
  • Develop and participate in orientation/in-service training programs related to residents’ needs.
  • Maintain confidentiality of all resident information and comply with HIPAA regulations.
