The primary purpose of your job position is to assist in planning, developing, organizing, implementing, evaluating, and directing our facility's social service programs in accordance with current existing federal, state, and local standards, as well as our established policies and procedures, to assure that the medically related emotional and social needs of the resident are met/maintained on an individual basis. In addition to planning the discharge to the lower level of care and include psychosocial well-being and care planning if needed based on the assessment of the resident. Requirements

  • Must be able to read, write, speak, and understand the English language.
  • Must possess the ability to make independent decisions when circumstances warrant such action.
  • Must demonstrate the knowledge and skills necessary to provide care appropriate to the age-related needs of the residents served.
  • Must be a supportive team member, contribute to and be an example of teamwork and team concept.
  • Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public.
  • Must possess leadership ability and willingness to work harmoniously with other personnel.
  • Must have patience, tact, cheerful disposition, and enthusiasm.
  • This position requires the successful completion of a background check, pre-employment physical, and drug screening as part of the hiring process.

Nice To Haves

  • Experience working with local insurance representatives preferred.
  • Knowledge of electronic medical record system Point Click Care (PCC) Preferred.
  • Experience as a Social Services Assistant preferred.

Responsibilities

  • Assist the Social Service Director/Consultant in the planning, developing, organizing, implementing, evaluating, and directing of the social service programs of this facility.
  • Meet with administration, medical and nursing staff, and other related departments in planning social services, as directed.
  • Assist in the development, administering, and coordinating of department policies and procedures.
  • Review department policies and procedures, at least annually, and participate in making recommended changes.
  • Assist in developing and implementing policies and procedures for identifying the medically related social and emotional needs of the resident.
  • Participate in community planning related to the interests of the facility and the services and needs of the resident and family.
