About The Position

Transitions Healthcare North Huntingdon is seeking a Social Services Assistant. The individual’s primary purpose in this position is to implement social service interventions that address the psychosocial needs and enhance the quality of life for residents and/or their responsible parties while linking social supports for the physical, mental, and financial well-being of residents in, and post discharge from the facility. Social Services Assistant Job Highlights: · Greets new residents and family upon admission to the facility and answers any questions, provides comfort and support for adjustment issues, and obtains a social history of the new resident. · As part of the admission process, meets with residents and/or responsible parties to review policies on advanced directives, living wills, DNR status, and resident rights and discharge planning. Implements and completes documents as appropriate with residents and/or their responsible parties. · Evaluates psychosocial and other treatment needs for assigned residents and establishes goals and develops treatment programs. · Interacts with residents and family to encourage participation and resolve issues in a timely fashion and timely documents all problems, interventions, and results. · Ensure that assessments are initiated and completed according to the resident’s level of care and complete assessments, MDS and care plans according to regulatory guidelines and facility policy. · Attend and participate in care plan meetings with emphasis on medically related social services including discharge planning, DME, supplies and services. · Develops and monitors behavioral intervention programs in consultation with professional and direct care staff for assigned residents. · Develops and implements discharge plans as appropriate for residents returning to the community. · Works collaboratively with psychiatric services as needed. · Serve as primary liaison for residents and/or their responsible parties regarding communication and development of interdisciplinary treatment and/or discharge planning. · Maintains current listing of all residents, POAs, guardians, responsible party’s names, addresses and phone numbers. Identifies residents who need POAs or guardians and initiates actions to encourage securing of appropriate POAs or guardians. · Monitors and timely documents progress toward the Care Plan goals for assigned residents. · Oversee, log, track, and report grievances/concerns on a timely basis. Reports grievances to the Department Director affected by said grievance for timely resolution. · Actively participates in daily Interdisciplinary Team meetings to ensure needs of resident and families are being met.

Requirements

  • High School Diploma preferred and experience in Case Management preferably in Long Term Care.
  • Excellent computer skills in a Microsoft Windows environment
