White Blossom Care Center is seeking a compassionate and experienced Social Services Assistant to join our dedicated care team. Requirements

  • Skilled Nursing Facility experience required
  • Knowledge of state & federal regulations related to long‑term care social services
  • Strong communication, documentation, and organizational skills
  • Compassionate, resident-focused, and able to work collaboratively within a care team
  • High school diploma required

Responsibilities

  • Support the Social Services Director in planning and delivering resident-centered services
  • Assist residents and families with care planning, transitions, and psychosocial needs
  • Help complete required documentation, assessments, and progress notes in compliance with state and federal regulations
  • Communicate with interdisciplinary team members to ensure resident needs are met
  • Advocate for resident rights and support their emotional well‑being
  • Participate in discharge planning and community resource coordination
