About The Position

The Social Services Assistant at White Oak of York provides essential social services support to residents and their families, ensuring adherence to state, federal, and facility standards. This role involves active participation in admissions, care planning, and discharge processes, as well as meticulous maintenance of resident social histories and progress notes. The assistant acts as a crucial liaison among residents, families, staff, hospitals, and external agencies, offering emotional support, particularly during crises. Responsibilities also include assisting residents with practical needs such as medical appointments, transportation, equipment, and personal requests. The position requires monitoring changes in resident conditions, reporting incidents, and providing assistance during emergencies, all while strictly following HIPAA guidelines and facility policies. Requirements

  • Bachelor’s degree in Social Work or a related Human Services field.
  • Ability to assist residents physically, including pushing wheelchairs and repositioning.

Responsibilities

  • Provide social services support to residents and families in compliance with state, federal, and facility standards
  • Participate in admissions, care planning, and discharge planning.
  • Complete and maintain resident social histories and progress notes.
  • Serve as a liaison between residents, families, staff, hospitals, and outside agencies.
  • Provide emotional support to residents and families, including during crises.
  • Assist with medical appointments, transportation, equipment needs, and personal requests.
  • Monitor changes in resident condition, report incidents, and assist during emergencies.
  • Follow HIPAA guidelines and facility policies.
  • Perform other duties as assigned.
