About The Position
White Blossom Care Center is seeking a compassionate and experienced Social Services Assistant to join our dedicated care team. If you have Skilled Nursing Facility (SNF) experience and a passion for supporting residents and families, we want to hear from you!
Requirements
- Skilled Nursing Facility experience required
- Knowledge of state & federal regulations related to long‑term care social services
- Strong communication, documentation, and organizational skills
- Compassionate, resident-focused, and able to work collaboratively within a care team
- High school diploma required
Nice To Haves
- related coursework or certification preferred
Responsibilities
- Support the Social Services Director in planning and delivering resident-centered services
- Assist residents and families with care planning, transitions, and psychosocial needs
- Help complete required documentation, assessments, and progress notes in compliance with state and federal regulations
- Communicate with interdisciplinary team members to ensure resident needs are met
- Advocate for resident rights and support their emotional well‑being
- Participate in discharge planning and community resource coordination
