About The Position

Altoona Nursing and Rehab is seeking a Social Worker Assistant. In this role, you will work with residents to identify their psycho-social, mental, and emotional needs, while providing, developing, and aiding in the access of services to meet those needs. You will also provide guidance to facility staff in matters of resident advocacy, protection, and promotion of residents' rights. As a Social Worker, you will play a key role in supporting residents and families, collaborating with department directors, medical professionals, and community agencies.

Requirements

• Social Service Designee certification
• Criminal background check
• Experience working in a nursing home setting is preferred

Responsibilities

• Identify residents' mental and emotional needs and assist in fulfilling those needs.
• Assist in care conferences, admissions, and other assigned duties.
• Collaborate with the management team and residents' medical team to ensure all resident needs are met physically and emotionally.
• Plan, develop, and organize social service programs that meet resident-directed goals.
• Provide intensive and/or continuing counseling and support to residents for their particular needs.
• Collaborate with the team to meet individual psycho-social needs of residents.
• Determine the nature of residents' situations by interviewing them and assessing medical, psychological, emotional, and social information.
• Establish a course of action by exploring options and setting goals with residents.
• Obtain assistance for residents by referring them to community resources and arranging appointments.
• Maintain records of cases by documenting residents' situations and actions.
• Monitor planned actions through periodic follow-up.
• Maintain operations by following policies and procedures, participating in quality reviews, and reporting needed changes.
• Comply with federal, state, and local legal requirements by studying existing and new legislation, enforcing adherence to requirements, and advising management on needed actions.
• Maintain client confidence and protect operations by keeping information confidential.
• Contribute to team effort by accomplishing related results as needed.
