About The Position

The Social Services Assistant plays a crucial role in the skilled nursing facility by assisting the Director in planning, developing, organizing, implementing, evaluating, and directing social service programs. This position is responsible for discharge planning, developing social care plans, and conducting resident assessments, while also providing essential information to residents and families regarding financial assistance programs.

Requirements

  • Minimum of one year experience in social services.
  • Experience in skilled nursing is highly preferred.
  • Working knowledge of Point Click Care and MDS is highly preferred.

Responsibilities

  • Assist the Director in planning, developing, organizing, implementing, evaluating, and directing social service programs.
  • Plan and develop discharge planning and social care plans.
  • Conduct resident assessments and implement care plans.
  • Provide information to residents and families about Medicare/Medicaid and other financial assistance programs.
  • Direct the social service department with authority and accountability.
  • Ensure social service progress notes are informative and descriptive of services provided and residents' responses.
  • Maintain a reference library of written materials, laws, and standards necessary for compliance with current regulations.
