About The Position
The Social Services Assistant plays a crucial role in enhancing the quality of life for residents in our skilled nursing facility. This position involves supporting the Social Services Director in delivering high-quality care and services tailored to the needs of our residents. The Social Services Assistant will be responsible for assisting in the planning, development, organization, implementation, and evaluation of social services programs that cater to the diverse needs of our customers. This role is essential in ensuring that residents receive the necessary counseling and support services, which are vital for their emotional and physical well-being. In this position, the Social Services Assistant will identify the medical-related social needs of residents and provide appropriate services to address both individual and collective needs. Maintaining accurate records related to the social work needs and care of customers is a key responsibility. The Assistant will also consult with the Director of Social Services and collaborate with other department heads to address interdisciplinary issues and ensure the maintenance of appropriate records. The Social Services Assistant will work directly with residents and their legal representatives, helping them navigate personal and environmental challenges that may arise due to the residents' physical or emotional conditions. This role requires a compassionate and dedicated individual who is committed to making a positive impact in the lives of our residents and their families.
