About The Position

The Social Service Director/Consultant is responsible for planning, developing, organizing, implementing, evaluating, and directing the social service programs of the facility. This role involves collaboration with administration, medical and nursing staff, and other departments to ensure the social services meet the needs of residents and their families. The position also includes administrative duties, community planning, and providing support to residents to achieve their highest level of self-care and well-being.

Requirements

  • High school diploma or equivalent required; Associate or Bachelor's Degree preferred.
  • Prior health care experience preferred.
  • Ability to read and interpret business and professional documents.
  • Ability to write reports and business correspondence.
  • Ability to present information effectively to managers and employees.
  • Mathematical skills to apply concepts like fractions and percentages to practical situations.
  • Reasoning ability to solve practical problems and interpret instructions.

Nice To Haves

  • Experience in social services or healthcare settings.
  • Knowledge of Medicare/Medicaid and financial assistance programs.

Responsibilities

  • Assist in planning, developing, and implementing social service programs.
  • Meet with administration and staff to coordinate social services.
  • Review and recommend changes to department policies and procedures annually.
  • Develop policies for identifying residents' social and emotional needs.
  • Participate in community planning related to facility interests and resident needs.
  • Assist residents in achieving self-care and independence.
  • Provide medically related social services to maintain residents' well-being.
  • Complete necessary administrative forms and reports for the Director.
  • Inform residents and families about financial assistance programs.
  • Consult with staff and community agencies to address residents' needs.
  • Maintain regular Social Service progress notes.
  • Coordinate social service activities with other departments.
  • Assist residents and families with emotional problems related to illness and institutional life.
  • Evaluate social and family information for treatment planning.
  • Meet regularly with department personnel to improve services.
  • Obtain resources from community agencies for residents' needs.
  • Assist in scheduling outpatient appointments and ancillary services.
  • Support discharge planning with appropriate agencies and referrals.
  • Develop and maintain rapport with other departments and agencies.
  • Participate in orientation and training programs for staff.
  • Assist in developing the department's budget.
  • Report suspected fraud and maintain confidentiality of resident information.
