To provide professional social services, including, but not limited to, psychosocial assessment, patient advocacy, discharge planning, resource management, crisis intervention, corn munity education, and supportive counseling, designed to meet the varying psychosocial needs of patients and their families as these issues relate to the current hospitalization or subsequent transition from the acute hospital setting. To provide professional social services to high-risk outpatients, including those in the Cancer C are Resource Center, Diabetes Management Center, Infusion Center, Outpatient Imaging Center, and Outpatient Pulmonary Rehabilitation.
