About The Position

Park View , a Cassia community, is hiring a Social Service Designee to join our team! This flexible, 16-hour-per-week position is a wonderful opportunity for someone who is passionate about supporting older adults and their families while maintaining work-life balance. As the Social Service Designee , you will advocate for residents and families to ensure they receive high-quality care and services. You will provide social support, coordinate individualized plans, complete assessments, assist with admission agreements, and serve as a liaison between families, staff, and outside agencies. The ideal candidate has a degree in Social Work or Human Services or equivalent experience, strong communication skills, and long-term care experience preferred. Position Type: Part-Time (16 hours per week) Shifts Available: Days: Flexible schedule Wage Range: $23 to $28 / hour depending on experience Location: 200 Park Ln, Buffalo, MN 55313

Requirements

  • Degree in Social Work or Human Services preferred; or equivalent experience.
  • Exceptional interpersonal, oral and written communication skills.
  • Ability to provide compassionate emotional and behavioral health support to residents and families.

Responsibilities

  • Provide valuable social services to residents and their families in medical, emotional, physical, or psychological concerns.
  • Assist the team in coordinating social services for the facility, individualized plans for the residents, medical equipment, counseling or other follow-up treatment plans.
  • Act as liaison between families, staff, and various agencies.
  • Assist with the completion of Admission Agreements.
  • Conduct and document BIMS, PHQ-9, and Psychosocial assessments.
  • Provides resources and referrals to staff, residents, and family members.
  • Provide additional tasks as required.
