About The Position

The Social Service Designee Assistant plays a crucial role in supporting the Social Service department by performing clerical duties and assisting in the management of residents' emotional and social needs. This position involves working closely with the Social Service Director to implement and evaluate social service programs while ensuring compliance with relevant standards and policies.

Requirements

• Must possess a Social Service Designee certificate.
• Prefer at least one year of experience in a social services department, skilled nursing care facility, or related medical facility.
• Must be able to read, write, speak, and understand the English language.
• Must pass a pre-employment background check and drug test.

Responsibilities

• Assist the Social Service Director/Consultant in planning, developing, organizing, implementing, evaluating, and directing social service programs.
• Participate in discharge planning and the development and implementation of social care plans and resident programs.
• Interview residents and families in a private setting as necessary.
• Perform administrative tasks such as completing necessary forms and reports, and submitting them to the Administrator.
• Involve residents and families in planning social service programs when possible.
• Assist in arranging transportation to other facilities when necessary.
• Refer residents and families to appropriate social service agencies when needed.
• Provide information to residents and families regarding Medicare/Medicaid and other financial assistance programs.
• Offer consolation to staff and community agencies to address residents' needs and problems through social service program development.
• Perform other duties as deemed necessary by the Administrator.
