About The Position

The Social Service Counselor 1-3 position is part of the Home and Community Based Services program in Slidell, Louisiana. Requirements

  • A baccalaureate degree in a human service-related field.
  • Certification or eligibility for certification as a Counselor in Training (CIT) or a current practice credential as a Certified Addiction Counselor (CAC), Registered Addiction Counselor (RAC), or Licensed Addictive Counselor (LAC).
  • For prevention services, certification or eligibility for certification as a Prevention Specialist in Training (PSIT) or a current practice credential as a Certified Prevention Professional (CPP), Registered Prevention Professional (RPP), or Licensed Prevention Professional (LPP).
  • Six years of full-time work experience may substitute for the required degree.

Responsibilities

  • Provide direct social services and counseling to clients in the Home and Community Based Services program.
  • Manage specialized caseloads for individuals with long-term disabilities.
  • Complete initial and annual housing assessments for clients.
  • Assist clients in establishing contacts and maintaining a safe home environment.
  • Develop daily living skills for clients.
  • Participate in planning and managing the Individual Service Plan (ISP).
  • Serve as a liaison between clients, families, and service providers.
  • Link clients to appropriate services offered by the agency.
  • Monitor service provisions through observations and visits.
