The Social Service Coordinator will play a crucial role in providing residents with the opportunity to age in place, despite facing challenges such as declining health, poverty, and emotional stress. This will be achieved by linking residents to the community and supportive services that enable them to continue living independently. The Social Service Coordinator will be responsible for developing strong resident retention programs and identifying any abnormal physical, behavioral, or unit conditions, while seeking effective solutions for these issues. The position operates within a framework that complies with Fair Housing laws and Equal Employment Opportunity, ensuring that all residents receive equitable support and services. In this role, the coordinator will interact with residents to monitor their conditions and needs, conduct assessments and reassessments, and manage cases and crises. They will also be tasked with developing and managing programs aimed at enhancing the quality of life for residents and increasing cognitive stimulation. Reporting, documentation, and recordkeeping are essential components of this position, as is community building, which involves connecting residents with local resources and making referrals to appropriate agencies. The coordinator will also be responsible for creating a monthly calendar of educational programs for residents and attending corporate meetings with senior service providers to stay informed and engaged with community resources.
