Key Position Details: 1.0 FTE (80 hours per two-week pay period) 8-hour day shifts No weekends Job Description: Reviews referrals for disability program eligibility, coordinates client appointments with Social Security staff, prepares application documentation, and acts as a liaison with the Social Security Administration. Key duties include detailed phone screenings, managing client communications, obtaining and organizing medical records, handling correspondence for the department, and participating in staff development and training.

Requirements

  • Must be 18 years of age with education and/or experience needed to meet required functional competencies as listed on the job description
  • 2+ years Social service, human or legal services, or case management experience, including proficiency with Microsoft Office functions

Nice To Haves

  • High school diploma or GED
  • Bachelor's degree in social work, communications, or related field
  • 1+ year Social Security Disability experience

Responsibilities

  • Social Security Advocacy Manages a high volume of phone interactions, including incoming client calls and proactive outreach to clients, agencies, and other stakeholders involved in the Social Security application process.
  • Conducts high-level phone screenings to determine initial eligibility for Social Security Disability.
  • Gathers essential medical and financial information from clients.
  • Addresses sensitive or personal topics with professionalism, respect, and empathy while maintaining clear, consistent communication and answering client questions throughout the process.
  • Routinely collaborates with the Social Security Administration to provide necessary information, complete status checks, and support efficient claim processing.
  • Uses Prevail case management software to maintain accurate, detailed notes and keep all client files updated, organized, and compliant.
  • Administrative Work Arranges appointments for Social Security staff.
  • Requests, organizes, and submits medical records to the Social Security Administration.
  • Responsible for scanning all incoming mail into clients’ files and preparing outgoing mail requests for the department.
  • Updates Social Security cases and complete medical record invoices.
  • Communicates effectively and professionally both in writing and speech when working with clients, medical professionals, and SSA representatives.
  • Staff Development Supports clients through challenging interactions with compassion and understanding, maintaining a calm and respectful approach.
  • Demonstrates adaptability and flexibility in managing shifting priorities and responsibilities throughout the day.
  • Assists with the education of Social Security to other hospital departments.
  • Attends training through the State of Minnesota for the Social Security Advocacy Program.
  • Special projects as directed by leadership.
  • Other duties as assigned.
