PURPOSE OF POSITION: A Social Case Worker I at Catholic Charities: Identifies clients who are eligible for benefits. Assists clients in the benefits application process. Monitors and assesses client progress towards identified goals. Refers and facilitates connection of participants to needed professional and human service agencies and initiates follow-up services as needed. Coordinates case conferences with agencies or persons involved with the participant’s self-sufficiency efforts as necessary. Authorizes services of the organization based upon assessment of need and availability of resources. Maintains accurate case records; documents client contacts and progress in order to ensure accountability. Inputs required data information for statistical and monthly reports. Provides feedback on systemic barriers impeding the success of the household or program. Participates as a positive team member through communication, cooperation, information sharing and problem solving. Demonstrates skills and sensitivity to the cultural/linguistic differences of staff, clients and community. Participates in staff training as required. Plans and provides training for community agencies and groups as needed. Completes or provides input to reports documenting program activities. Reflects Catholic Charities’ commitment to treating all persons with dignity and respect Uses creativity and innovation in program development and service delivery. Maintains confidentiality of client and agency information. Regular and predictable attendance.
